The Ministry of Transportation asks some drivers for a medical report, including commercial drivers at set intervals and drivers whose health has been flagged for review. In Ontario, a Nurse Practitioner can complete these reports.
Who usually needs one
- Class A, B, C, D, E or F licence holders when the ministry asks for a medical
- Drivers who received a ministry letter asking for a medical report
- Drivers returning to driving after a health change, when the ministry requests it
What happens at the visit
The visit is about 60 minutes, in clinic. A nurse reviews your health history and medications and takes your vital signs. The Nurse Practitioner then completes the examination the form requires and fills in the ministry form. If the ministry needs more information, such as a specialist report, we’ll tell you what to arrange.
Please bring
- The ministry letter or form
- Your driver’s licence
- Glasses, contact lenses or hearing aids you use
- A list of your medications
- Any specialist reports linked to the condition named in the letter (for example diabetes, heart, sleep apnea or seizure records)

Please note. The ministry decides on licence status. Ontario law also requires physicians and nurse practitioners to report certain medical conditions to the ministry; we’ll explain if that applies to you.
