Good to know
- A note or simple form uses an existing assessment. If a new assessment is needed, we tell you the fee before you book.
- Many notes and simple forms can be done by secure video for people in Ontario. Forms that need an examination are done in clinic.
- Ontario employers can’t require a note from a health professional for the job-protected sick leave days under the Employment Standards Act.
- We can’t guarantee that an employer, insurer or school will accept a form or approve a claim.

To send documents before your visit, reply to the secure message we send through your Jane account after you book. Please don’t send health information or documents by email or WhatsApp.
