For people who find it hard to get to a clinic, our nurses, Registered Social Worker and Nurse Practitioner can visit at home in Sarnia-Lambton. Care is planned around your needs and works alongside your existing care team.
What home visits can include
- Nursing visits: monitoring, wound care, injections and education
- Medication reviews, with family or caregivers present
- Nurse Practitioner assessment for people who can’t easily travel
- Social work support for caregivers, planning and adjusting to illness
- Comfort-focused and palliative support, coordinated with your care team
How to arrange a visit
- Call 548-997-9797 or message us on WhatsApp.
- We’ll have a short screening call to confirm a home visit suits your needs and location.
- Fees and any travel-zone fee are confirmed before the visit.
- On the day, please secure pets, keep the care area clear and well lit, and let us know who will be present.
Good to know. Home visits aren’t for urgent care; in an emergency, call 911. If you may qualify for publicly funded home care, Ontario Health atHome can assess you, and we’re happy to work alongside them.
